Clinical Team Leader

Our Client is One of UK's leading Healthcare Providers and they are currently seeking for a motivated Clinical Team Leader.

You will be working in a highly reputed service within the Burton-On-Trent.


You will work alongside the Community Matron Team, the Care Coordination Centre, community nursing team, the therapy teams and specialist nurses to support the East Staffs Improving lives model of care.

The team of specialists aim to work closely with other health care providers to reduce hospital admissions and empower patients to proactively manage their long term condition.


As a Clinical Team Leader your day to day duties include:

·         Providing autonomous nursing practices to assess, investigate, diagnose, treat and refer patients as appropriate on clinical visits

·         Always acting within the parameters of the NMC Code of Professional Conduct.

·         Organising and developing nursing care services for the patients of the service

·         Contributing to the provision of multidisciplinary education and training within the service, local training programme and providing experiential learning opportunities, mentoring and clinical learning opportunities

·         Carrying out home visits for further patient assessment when required

·         Triage and appropriately respond to urgent calls for same day assessments for patients within nursing or residential homes to ensure effective assessment and management of a patients condition to improve the patients outcomes and reduce avoidable hospital admissions and attendances

·         Work collaboratively with our colleagues who are based at the local trust

·         To work closely with other members of the team, health and social care partners and GPs

·         Attend MDT meetings to discuss patients that are at risk of admissions and signpost where appropriate as required – patient led if appropriate for Rapid Response


An ideal Clinical Team Leader must be a hardworking, confident individual and must have these skills as they are beneficial for this position:

·         First level Registered Nurse

·         Adult physical assessment qualification 

·         Clinical experience - working in primary healthcare for a minimum of 5 years at a senior nurse level

·         Teaching and Assessing qualification (minimum ENB 998 or equivalent)

·         Experience of working with chronic disease management.

·         Excellent communication, organisation and interpersonal skills

·         Evidence of continuing professional development 

·         Full driving licence and access to a car 

·         Ability to make complex decisions and problem solving abilities

·         Ability to manage own time effectively

·         Ability to work to tight deadlines 

·         Ability to make clinical decisions and take appropriate action 

·         Motivational skills 

·         Basic IT skills


The successful Clinical Team Leader will receive an excellent annual band 7 salary of £33,222-£38,765.

This exciting position is Monday - Friday from 9-5.
