Brighter Futures, based in Stoke on Trent, is working in partnership with North Staffordshire GP Federation (NSGPF) to facilitate and support our emerging Primary Care Networks (PCNs) with an exciting opportunity for a team of up to 9 enthusiastic Social Prescriber Link Workers, and an Operational Manager to develop the social prescribing service in North Staffordshire.

The NHS future long term plan for general practice encourages the development of these new and exciting roles focussing on the capability and workforce capacity.  The role of social prescribers within General Practice is recognised as vital to create resilient primary care teams, able to provide an integrated, patient centred service.

You will provide support and cover for the staff team during, holidays and sickness as well as supporting staff members that may have a larger case load.

You will provide 1:1 personalised support to people who are referred to you by team members and the various primary care networks (PCN’s) 

Within this position you will directly empower people to take control of their health and wellbeing by giving time to focus on ‘what matters to me’.

Social prescribing will build trusting relationships with people, create a shared personalised care and support plan and connect them to community groups and services.

You will be based in GP practices supporting your PCN so your own transport is essential.

This role helps people to work on their wider health and wellbeing, specifically wider determinants of their health, such as debt, poor housing and physical inactivity, as well as other lifestyle issues and low-level mental health concerns by increasing people’s active involvement with their local communities.

This approach particularly helps people with long term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing. 

You must be a good listener, have time for people and be committed to supporting local communities to care for each other.

You should have experience of working positively with people facing complex social and emotional challenges.

You will have great interpersonal skills in supporting people, community groups and local organisations. 

You will need to be able to manage your own caseload, be self-motivated and work with minimum supervision as well as working collaboratively with the rest of the Social Prescribing team to discuss and give feedback on all referrals generated.

You will also be required to attend regular meetings reporting to brighter futures with a shared clinical supervision for the PCN, and to share knowledge, experience and best practice for the benefit of all partners.
