Sirona Medical are looking to recruit a Social Care Assessor based in Merseyside.

Job Role: This particular role is based within Adult Social Services.

The Social Care Assessor will work alongside members of the wider multi-disciplinary team (MDT), which includes psychiatrists, CMHNs, health nurses, occupational therapists, GP.

Sirona Medical will be providing the Social Care Assessor to a Local Authority in Merseyside.



To be sucessful for this role, Sirona Medical are looking for applicants to possess the below skills/experience;
Able to complete Care Act Assessments, risk assessments and asset-based support planning.

Undertaking reviews Experienced in handling a lively case load and be able to work autonomously and within a multi-disciplinary team.

The role is one Social Care Assessor Monday to Friday 09:00am - 17.00 pm Monday to Friday. 

Previous experience in completing Care Act Assessments and undertaking reviews are essential to apply for this role.
Enhanced DBS is required for this role.
