Sirona Medical are looking to Social Worker for the East Locality Team in West Midlands.

Job Role: This particular role is based within Wallsall.

The Social Worker will complete Care Act assessments, support plans and reviews of package of care and care home placements.

Also completed carers assessments and support plan.

To use mosaic IT system to record all work including case notes, assessments, reviews and support plans.

To work on duty to screen new referrals and respond to telephone calls from services users, carers, and other professionals.

To complete safeguarding section 42’s and Mental capacity assessments.

Sirona Medical will be providing the Social Worker to the East Locality Team.

This is a full time vacancy, 37 hours per week for 3 months initially.

As a Social Worker, you must have a degree in Social Work with a minimum 2 years post qualified experience.
To act in accordance with Social Work England registration requirements and Code of Conduct for exercising accountability and professional practice.



Previous experience and/or qualifications in Social Work are essential to apply for this role, recent experience in working in similar team is also essential.
